This information collection is required to process all official Continental United States and Outside the Continental United States Exchange Permanent Change of Station requests for Exchange employees and their dependents/family members. The data collected assists the Exchange Human Resources Travel personnel in determining eligibility of the employee and/or dependents to travel, obtain passports, visas, and authorizing any travel expense reimbursement.
