SUMMARY: This is a Notice of the Presidential declaration of a major 
disaster for Public Assistance Only for the State of West Virginia 
(FEMA-4219-DR), dated 05/14/2015.
    Incident: Sever Storms, Flooding, Landslides, and Mudslides.
    Incident Period: 04/03/2015 through 04/05/2015.
    Effective Date: 05/14/2015.
    Physical Loan Application Deadline Date: 07/13/2015.
    Economic Injury (EIDL) Loan Application Deadline Date: 02/15/2016.

    The following areas have been determined to be adversely affected 
by the disaster:
